CVS Health is the largest pharmacy benefit manager in the US with 30% market share, followed by Evernorth (owned by Cigna and formerly Express Scripts) at 23%, and OptumRx (owned by UnitedHealth Group) at 23%. During the week, CVS Health held its annual Mother’s Day. (Links below.) After the Aetna transaction, CVS Health became a uniquely diversified organization with over $250 billion in revenue.

As expected, CVS Health management explained cost-cutting synergies expected from CVS Health and Aetna’s merger. It also envisioned a long-term vision for different parts of our empire to work together in new ways to create value and generate operating profit. CVS Health calls these new products and services “innovative initiatives.”

Cvs Health To Hire 15,000 Across The U.s. In Fourth Quarter

Below, I summarize the highlights of the investor day related to CVS Health’s overall strategy, reflect on the challenges facing the HealthHUB store concept, and outline trends and trends affecting the company’s key business segments.

I applaud CVS Health Management for their willingness to take a bold step forward. But will its transformation grow at the speed and efficiency needed to generate higher returns for CVS Health and its investors? In the future, how much margin will the company pay to sustain the new business combination?

As always, I recommend that you read the original source material yourself. Here are links to the CVS Health 2019 Investment Day:

INCLUDING, I reviewed some of the PBM business issues related to CVS, Express Scripts, and the evolution of the PBM business model.

The CVS Health-Aetna transaction was announced in December 2017. At the time, I thought about how the company could find true synergy from its vast operations and the diversity of its corporate culture. See the CVS-Aetna deal: Impact on five industries and drug channels.

CVS Health expects annual operating revenue of $850 million by 2022. He said the reforms would generate $2.5 million in additional revenue over the “long term” — an unspecified period after 2022. More information can be found in the section Building a consumer-centric health company.

I don’t see how expats can attribute “increased” operating income to change rather than the many business developments and changes that may occur over the next five or so years. I believe CVS Health will provide detailed information on operational revenue data for future products and services.

The HealthHUB store format is at the heart of the company’s consumer-focused transformation initiatives. These locations will offer a wide range of health care services, including expanded services from existing MinuteClinics. To do this, CVS Healthcare must change not only its business, but also customer perceptions, all while fending off competition from the growing urgent care clinic industry.

A useful summary of the HealthHub concept and services: HealthHub: Accelerating our mission to transform the healthcare experience.

In early 2019, CVS opened its first three HealthHUB store locations in Texas. CVS says these HealthHUBs can handle 80% of the services in a typical primary care practice. Plans to expand to Philadelphia (yay!), New Jersey, Atlanta and Tampa have been announced. It plans to have around 1,500 HealthHUB retail outlets by the end of 2021. Many of these sites will be renovated and expanded MinuteClinic locations.

The CVS Health-Aetna combination is trying to fulfill the dream of affordable, accessible health care that promises to come with pharmacy-linked retail clinics.

As recently as December 2014, CVS said it will have 1,500 minute clinics by 2017. But for 2018, MinuteClinic only had 1,100 spots. The total number of MinuteClinic locations has decreased over the years. See section 1.4.1 for more information on the retail clinic market. As far as I know, CVS Health has not overcome the obstacles that limit the growth of retail clinics. I believe that many consumers do not see retail stores as a source of health care. However, operating HealthHUBs requires a significant shift in consumer perceptions.

CVS Health will compete with health care providers that currently offer many of these services. Check out the American Medical Association’s (AMA) strong opposition to the CVS-Aetna combination. (Check out the AMA’s 141-page critique of the deal, if you can.)

In a recent editorial, the Wall Street Journal praised CVS Health’s ability to compete with suppliers. WSJ called for hospitals’ in-house labs and diagnostic tests, hospital prices to rise faster than other parts of health care, and referrals to physicians from the 340B drug pricing program to distort the market. As mentioned above:

“[T]he Aetna-CVS deal drew opposition from the AMA because nurse practitioners working in retail clinics could provide routine care at a lower cost than doctors working in hospitals.” As a result, high-cost providers don’t sit back while CVS Health solves problems.

Cvs Health Company Culture

The Urgent Care Association (UCA) estimates that hospitals and other providers operated approximately 7,700 urgent care clinics in 2017. UCA defines an urgent care center as “a medical clinic specially equipped to diagnose and treat life- or limb-threatening illnesses and injuries. Urgent care centers are augmented by on-site radiology and laboratory services and are freestanding or hospital-based. From the emergency department Does he work elsewhere? Does he know you?

The ambulance market has rapidly become saturated with independent and hospital/IDN operators. If CVS Health can’t move forward as quickly as expected, it may acquire one or more independent urgent care operators to begin its transformation. I would frown upon such a move.

Here is a summary of the retail/LTC and pharmacy services segments. These industry trends will be familiar to readers of the Drug Channel or our annual manufacturing reports.

The Complete Investment Facility has lots of other interesting content, specialty drugs, and general perspectives on the pharma/PBM market.

CVS Health’s size and importance make it very influential in the US drug channel. It’s an open question whether the company is too big and too weak to work effectively and understand. Will disruptions of scale and scope hinder management’s efforts to be innovative and flexible?

For investors, CVS Health has become a show-me story. Its stock has fallen more than 50% over the past four years, suggesting investors are still not convinced. Are you
